Syllabus Acknowledgement Form
 

Purpose: To officially verify a mutual understanding between students and the instructor with regard to the Course Syllabus.

Instructions:
STEP 1 - Read this "Syllabus Acknowledgement Form" - (Refer to the Syllabus IF necessary)
STEP 2 - Copy and paste the "Syllabus Acknowledgement Form" into your email window
STEP 3 - Type in the date, your first name, last name, and initials (A parent's signature - IF you are a high school student)
STEP 4 - E-mail this document to Donna Rebadow

    Syllabus Acknowledgement:

  1. I understand that this is NOT a self-paced course.

  2. I understand that there are deadlines for each Assignment, Paper, Discussion Topic, Test and Final Exam.

  3. I understand the Time Commitments and Course Requirements and Semester Schedule for this Course.

  4. I understand the Policy for SUBMITTING WORK PAST THE STATED DEADLINES.

  5. I understand that I should have a separate and/or "new" email address just for this course.

  6. I understand that I am responsible for keeping copies of all my work and e-mail correspondence for this course.

  7. I understand that Plagiarism (the unacknowledged use of information discovered and reported by others or the use of their exact words, copied verbatim) is a serious offense and can result in failing this course; I will submit my own original work for this course.

  8. I understand that I will be REQUIRED to come to the PVCC Campus and take my Tests IF I experience technical difficulties while taking the Tests online.

  9. I understand that it is fully possible for a student to complete all work on time and still receive a failing grade for this course if the work submitted is not college-level.

  10. I understand that it is my responsibility to withdraw myself from this course. I understand the Withdrawal Policy.

  11. I understand the Grading Policy for this class.

  12. I understand that all Online Tests are timed and I am NOT to have an "open book" or notes with me while taking these tests.

  13. I understand that there are NO "Incompletes" given for this course.

  14. I understand all of the Policies in the course Syllabus and that I am responsible for all Maricopa Community College District and PVCC Campus Policies that apply to this course.

  15. I understand that I can find all the Maricopa Community College District and PVCC Campus Policies that apply to this course in the current College Catalog.

  16. I understand that I can contact Paradise Valley Community College and request that they mail me a copy of the current College Catalog OR I can pick one up at the PVCC campus.

  17. I understand that all provisions in this syllabus are subject to revision by the instructor. Such revisions, if any, will be sent to me via e-mail. It is my responsibility to make note of all such announcements concerning syllabus revisions and assignments.

  18. I understand that by e-mailing the "Syllabus Acknowledgement Form" (with my electronic signature) to my instructor that it becomes a "Contract" between the two of us.
